How to request medical records at ASU Health Services?

ASU Health Services Medical Records department maintains an electronic medical record system for all patients seen at the clinic. Prior to receiving copies of your records or authorizing records to be disclosed you will be asked to complete an authorization form and provide proof of your identification – a picture ID (driver’s license, ASU ID or passport).

You can request your records by filling out the needed request form. You can also request copies of your medical records through your Patient Portal.

Patient Portal Instructions:
  1. Sign in to My ASU.
  2. Select Campus Services, and your portal will automatically log in.  
  3. Select ASUHS Patient Portal.  
  4. Select My Chart.   
  5. Select Request Health Record.  


Visit ASU Health Services Medical Records or call (480) 965-3349 for more information regarding medical records or immunizations at ASU Health Services.

Please note: You may be charged a copying fee for your records if the records exceed the paper count of 10 pages. Medical records that are faxed or mailed for continuing care do not have a fee associated with the service.
