What do I do if I encounter a problem or error during my quiz, test, or exam?

If you encounter a problem or error with a quiz, test, or exam, there are several steps you should take:
  1. Document the problem or error to the best of your ability. Make sure you note the time, what occurred within your quiz, test, or exam (including any error messages), and take screenshots of the error. If you’re taking a proctored exam and receive an error message, you may need to take photos with your phone instead of a traditional screenshot. 
     
  2. Contact the ASU Experience Center to receive a ticket number as soon as possible and leave the error up on your screen for them to verify. They will provide troubleshooting steps or steps to prevent the issue in the future, in addition to documenting that you reached out to them regarding your quiz, test, or exam. If you do not contact them immediately, they may not be able to verify the issue but will still record that you contacted them.

  3. Contact your instructor to report the issue and determine your next steps for your quiz, test, or exam. Make sure you include all of the documentation you collected in steps 1 and 2, in addition to any information listed as required in your syllabus.
